Required Documents

K-12 Enrollment Documents

Students must be 5 years old on or before July 31

1. Birth certificate
2. Student's current immunization record
3. Parent/guardian photo ID (proof of guardianship, if required)
4. Previous school records – attendance, discipline, and grades
5. One proof of residency (see list below)

Pre-K Enrollment Documents

Students must be 3 or 4 years old on or before July 31 for Pre-K

1. Birth certificate
2. Student's current immunization record
3. Parent/guardian photo ID (proof of guardianship, if required)
4. One proof of residency (see list below)
5. Proof of household income:
   • Two current paystubs
   • Current TANF award letter
   • Current SSI verification
   • Unemployment letter
   • Current 1040A tax form with W-2's, (employer letter on company letterhead, notarized letter from employer)
   • Child support letter

Accepted Proof of Residency Documents

• Utility bill (water, gas, electric ONLY) within the last 30 days, no turnoff or disconnect notices

• Social services, social security statement within the last 30 days

• Paystub, paycheck, or bank statement within the last 30 days

• Mortgage statement within the last 30 days
• Other legal/court documents such as TANF or SNAP letters
• Valid property deed
• Signed residential lease agreement
• Current homeowner/rental insurance policy
• Current year real estate or personal property tax receipt

Residency Affidavit (click to download) – Must be notarized. KCPS provides notarization at no charge.

NOTE: If providing a residency affidavit, the Head of Household must provide one item from the Proof of Residency list #1 in their name AND the Parent/Guardian must provide one item from the Proof of Residency list #2 in their name.
